I use Machform: http://www.appnitro.com/ its cheap and at the time of purchase they were running a special of $200 for unlimited installs, that way you can use it for your different departments. Very easy to use, but has its limitations.

Google Docs/Forms works great for me because it can easily be shared with another Google account.
Just be careful about what personal information you receive.

Regarding Forms in OmniUpdate. Users can create forms within the WYSIWYG editor. There are tools for each type form element. No coding is required. The form can then be pointed to a form parser on your web server. Our new LDP (Live Delivery Platform) will automate this even further with the ability for your users to view and get reports on the content submitted via the forms they created. This is all within the user interface of OmniUpdate.
